Risk Assessment Form

Travel Vaccinationsglobal air travel

We are able to offer our patients Travel advice. We have a dedicated travel clinic run by our nurses usually held on the last Friday of the month. There is limited availability so it is advisable to book the appointment as early as possible.

Patients are advised to contact the surgery to book an appointment. You will then be asked to complete a travel risk assessment form which can be found on the surgery website. This can be completed on line and emailed back to reception or printed out and handed in. Your appointment must be booked before the assessment form is assessed.

The risk assessment form needs to be completed and returned at least 4 weeks before your appointment (and no less than 2 weeks)

We will not be able to asses last minute travellers and you will be redirected to a travel clinic.

We would recommend that you look at the “Travel well” leaflet from the link above.

Some vaccinations are not available on the NHS and will have to be paid for at the time of the appointment. Currently we are only able to accept cash, this may change in the future.

There is further information about countries and vaccinations required on the links below.
